Integrative Strategies to Optimize Outcomes for Bariatric Procedures Web Activity

Target Audience

This activity has been designed to meet the educational needs of medical directors, quality directors from health plans, group practices, employers, quality organizations, and other health systems as well as patient care team members involved with patient evaluation and follow up for bariatric procedures including: physicians, pharmacists, registered nurses, and case managers.

Statement of Need/Program Overview

Health care professionals, particularly those practicing in a PCMH setting, need instruction and tools to assist with identifying the right patients, the right treatment, and the right follow up, in order to ensure successful short and long term outcomes for medical services such as bariatric procedures. To address these needs and barriers, the proposed faculty will design and develop case study curriculum to address specific communication, collaboration, health information technology, follow-up requirements, and measurements with the goal of successful integration of patient services for bariatric procedures into the work flow of the medical home team. Through a series of integrated activities, this initiative will examine and recommend innovative strategies for patient communication, cost effectiveness, and follow-up requirements of available bariatric procedures with the ultimate goal of reducing the burden of disease caused by obesity. To further address this need, a “Tool Box” will be offered that can assist health care professionals with a PCMH approach to optimize outcomes with bariatric procedures.

Method of Participation

There are no fees for participating and receiving CME credit for this activity. During the period June 1, 2010 through December 1, 2011, participants must 1) read the learning objectives and faculty disclosures; 2) study the educational activity; 3) complete the online post test; 4) complete the online evaluation form. Upon successful completion of the post test (70% or better) and evaluation form, an online certificate will be made available immediately.

Upon successful completion of the activity evaluation form and a completed posttest with a score of 70% or better, a statement of credit will be mailed to you within 3 weeks.
